A family intensive is a concentrated, full-day therapeutic experience designed to do in one structured session what traditional weekly therapy can take months to accomplish. In a typical session, you spend the first chunk of time just getting back up to speed. With the intensive, Dr. Gaines has already reviewed every family member’s workbook, identified the patterns, and built a custom agenda before you ever walk in the room. You arrive with a game plan, not a blank slate.

The intensive is designed for families who are ready to stop avoiding and start moving. Most often, it’s an adult daughter who has done her own personal work and is now ready to bring her family into a deeper conversation. Parents who are willing to show up, even if they’re nervous, are also a strong fit. The common thread is willingness: everyone in the room has to be open to staying in it when things get uncomfortable. You don’t have to be perfect. You just have to be present.

This is one of the most common concerns, and it’s valid. The intensive is not a blame session, and it’s not mediation. Dr. Gaines creates a structured environment where every person in the room feels supported, including the parent. One father who participated came in afraid he’d be blamed for everything but left feeling heard and understood for the first time. Accountability is part of the work, but so is context, nuance, and making space for everyone’s experience.

This is the most common reason people hesitate, and it’s worth addressing directly. You don’t need everyone fully bought in before you reach out. Start with the consultation call — Dr. Gaines can help you think through how to have the conversation with your family member, what language tends to land, and whether the intensive is still viable if someone is resistant but willing to show up. Reluctance is not the same as refusal. Some of the most meaningful breakthroughs have happened with family members who came in skeptical and left with something they hadn’t expected to find.

The three-hour session is guided, structured, and built entirely around your family’s goals. Depending on what came up in your workbooks and pre-intensive meeting, Dr. Gaines may use role-playing, perspective-taking exercises, assertive communication practice, boundary-setting work, and real-time coaching. The schedule is flexible enough to slow down when something important surfaces or take small breaks when needed. You will not be sitting in silence wondering what comes next.

Both options are available. The pre-intensive meeting and post-intensive debrief are conducted virtually. The three-hour session is held in person at the Southfield office whenever possible, but a family Zoom meeting format is available for families where distance is a factor. One father-daughter pair successfully completed their intensive with the daughter flying in from out of state; another option for families spread across different cities is the virtual format from the start.

That’s more common than you’d think, and it’s completely okay. You don’t have to arrive with a diagnosis. The intake workbook and pre-intensive meeting are specifically designed to help Dr. Gaines draw out the patterns, even when families can’t name them directly. Most people can describe what keeps happening — the fight that repeats itself, the tension that never quite resolves, the thing no one will say out loud. That’s enough to work with.

The Family Intensive starts at $3,500 for the full package, billed at $350 per hour. That includes your initial consultation call, each family member’s intake workbook, Dr. Gaines’ workbook review and custom agenda preparation, the 90-minute pre-intensive meeting, the three-hour family intensive session, and the 90-minute post-intensive debrief. A 50% deposit is required to confirm. The intensive is private pay only and does not accept insurance. One family member serves as the single point of financial contact.

The post-intensive debrief is built into your package. That’s where Dr. Gaines shares clinical impressions, provides communication tools and strategies you practiced during the session, and outlines recommendations for what comes next. Some families find that one intensive gives them the clarity and reset they needed. Others choose to book a follow-up intensive to work through the next layer. The intensive is designed to create momentum, not dependency.

The consultation call is exactly where that question gets answered. Dr. Gaines will talk with you directly about your family’s situation, what you’re hoping to accomplish, and whether the intensive is the right vehicle to get you there. What makes a family a strong fit is not perfection — it’s willingness. Willingness to show up, to be honest, and to stay in the room when the conversation gets real.
